Acute pulmonary embolism (PE) remains a significant cause of cardiovascular morbidity and mortality, making prompt and accurate risk stratification essential for guiding management [1, 2]. Current guidelines, including those from the European Society of Cardiology (ESC), emphasize assessing PE severity to identify patients at high risk for adverse outcomes like hemodynamic instability or death [3, 4]. While tools incorporating clinical signs, imaging, and biomarkers like troponin are widely used, consistently identifying which patients will require advanced interventions remains a clinical challenge [1, 2, 5]. For instance, elevated troponin is a known predictor of poor outcomes, with one meta-analysis showing an odds ratio of 5.42 (95% CI: 4.35-6.83) for in-hospital mortality [1]. A recent retrospective study now provides a direct comparison of three scoring systems to clarify their specific strengths in predicting short-term outcomes.
