Early Radiographic Response Poorly Predicts Local Control in Brain Metastases

The management of brain metastases has increasingly shifted toward localized strategies like stereotactic radiosurgery (a precision radiation delivery method that targets tumors while sparing healthy tissue) to mitigate the neurocognitive risks associated with whole brain radiation [1, 2, 3]. In a randomized trial of 128 patients, the 12-month local tumor recurrence-free rate was 72% for stereotactic radiosurgery compared to 43% for observation alone (hazard ratio 0.46; 95% CI, 0.24 to 0.88; p=0.015) [2]. Despite these improvements, local recurrence remains a primary clinical challenge, with one-year failure rates reaching 40% in some cohorts [4, 5, 6]. Clinicians currently utilize standardized radiographic criteria to evaluate early treatment efficacy, yet the durability of these initial responses is often uncertain [7]. Furthermore, the integration of systemic therapies and the risk of radiation-induced necrosis (inflammatory tissue death caused by high-dose radiation), which occurs in 2% to 36% of re-irradiated patients, complicate the interpretation of post-treatment imaging [4, 8, 9]. To address this uncertainty, a new study evaluates whether early radiographic milestones accurately predict which patients will remain free from local recurrence over the long term.
