Acute kidney injury remains a frequent and severe complication following cardiac surgery, significantly impacting patient recovery and long-term survival. Despite the implementation of standardized care bundles that focus on hemodynamic optimization and the avoidance of nephrotoxic agents, the incidence of renal dysfunction remains high [1, 2]. Current clinical guidelines emphasize maintaining adequate perfusion pressures, yet the definition of adequate often varies across different critical care and surgical contexts [3, 4]. While interventions like glucose, insulin, and potassium infusions have been explored to mitigate postoperative complications, the primary focus for renal protection remains the management of intraoperative hemodynamics [5]. However, the persistent rate of injury suggests that traditional metrics like mean arterial pressure may not capture the full physiological picture of renal oxygen delivery.
