POT1 Mutations Drive Lymphoid Malignancy Through Ultralong Telomeres

While telomere shortening is a well-established driver of cellular senescence and age-related organ dysfunction, the clinical implications of excessively long telomeres have historically remained obscure [1]. Clinicians frequently encounter clonal hematopoiesis of indeterminate potential (a state where somatic mutations drive the expansion of blood cell clones), which is known to significantly increase the risk of hematologic malignancies and all-cause mortality [2, 3]. This process is often viewed through the lens of inflammaging (a state of chronic, systemic inflammation that accelerates immune cell senescence and impairs tissue homeostasis) [4]. However, emerging evidence suggests that genetic factors regulating DNA damage repair and telomere maintenance may dictate the trajectory of these clonal expansions [3, 5]. A recent study now clarifies how inherited defects in telomere regulation create a distinct predisposition to lymphoid cancers, offering physicians a new framework for identifying and monitoring high-risk families.
