Transradial Access Does Not Increase Radiation in Aneurysm Treatment

Transradial access, which involves entering the arterial system through the radial artery in the wrist, has rapidly gained traction in neuroendovascular surgery. This mirrors a transition in interventional cardiology, where the wrist is the preferred route for reducing major bleeding (odds ratio 0.52, 95% CI 0.38 to 0.71, p < 0.0001) and mortality [1, 2, 3]. In the neurointerventional suite, clinicians increasingly utilize this approach for mechanical thrombectomy and flow diversion (deploying endovascular stents to redirect blood flow away from an aneurysm) due to its low access-site complication rate of 0.8% (95% CI, 0.3% to 1.3%) and high technical success [4, 5, 6]. While the clinical benefits of the radial route are well documented, including a significantly shorter length of hospital stay (mean difference of -1.49 days, 95% CI -2.93 to -0.05, p = 0.04), questions remain regarding how this trajectory affects procedural efficiency and safety [7, 8, 9]. A critical component of this efficiency is the management of ionizing radiation, as complex intracranial interventions often require prolonged fluoroscopy times (continuous X-ray imaging used to guide catheters). A recent study now offers insights into how the choice between radial and femoral access impacts the total radiation dose, measured as dose area product (a metric reflecting both the intensity of the dose and the area of the X-ray beam), delivered during endovascular aneurysm repair [10].
