Psychological Factors Predict High-Impact Chronic Pain in Sickle Cell Disease

Chronic pain is the primary driver of morbidity in sickle cell disease, frequently resulting in profound functional impairment and diminished health-related quality of life [1, 2, 3]. While acute vaso-occlusive crises often dominate clinical attention, many patients transition into persistent pain states that lead to frequent hospitalizations (ranging from 0.5 to 27.9 per patient per year) and high healthcare resource utilization [3, 4]. Current management strategies are moving toward interdisciplinary models that integrate non-pharmacological approaches to mitigate the risks associated with long-term opioid therapy [5, 6, 2]. However, clinicians still struggle to identify which patients will progress to high-impact chronic pain, a phenotype characterized by the inability to participate in work, social, or self-care activities [4]. The Sickle Pain-Related Impact (SPiRIt) study, a prospective longitudinal analysis of 90 patients, found that 56% of participants remained in or transitioned into a high-risk state over six months, identifying pain catastrophizing (a cognitive pattern of magnifying the perceived threat of pain) as a key predictor of this trajectory [4].
