Skin Biopsies Differentiate Body-First and Brain-First Parkinson’s Disease

The clinical heterogeneity of Parkinson’s disease complicates prognostic counseling, as the definitive histopathological hallmark of misfolded alpha-synuclein aggregation often lacks reliable, non-invasive correlates. Traditional biofluid markers in the cerebrospinal fluid and saliva show significant variability, with total alpha-synuclein in the cerebrospinal fluid demonstrating a sensitivity of only 0.72 and a specificity of 0.65 [1, 2, 3]. Recent meta-analyses indicate that skin biopsies offer high diagnostic precision, particularly when using anti-phosphorylated alpha-synuclein antibodies, which have demonstrated a specificity of 1.00 (95% CI 0.98 to 1.00) in cutaneous tissues [4, 5]. Clinicians may find these cutaneous assays more actionable than enteric biopsies, as gut-based detection of phosphorylated alpha-synuclein yields a lower sensitivity of approximately 0.579 [6]. Identifying whether protein misfolding begins in the peripheral autonomic nerves or the central nervous system is essential for predicting phenoconversion, the transition from prodromal symptoms to a defined clinical syndrome, in patients with sleep disorders [7, 8]. A recent study characterizes these divergent pathological origins by mapping distinct cutaneous signatures across various disease phenotypes, offering a potential tool to stratify patients and tailor early interventions.
