Amitriptyline Not Superior to Placebo for Leprosy-Related Neuropathic Pain

Neuropathic pain remains a substantial clinical challenge, often diminishing patient quality of life and functional capacity [1, 2]. While first-line agents such as duloxetine, gabapentinoids, and tricyclic antidepressants are mainstays of treatment, their effectiveness is inconsistent across the diverse etiologies of neuropathic pain, from diabetic neuropathy to spinal cord injury [1, 3, 4, 5, 6, 7]. For example, duloxetine has demonstrated efficacy for painful diabetic neuropathy [5] and gabapentinoids for pain related to spinal cord injury [7, 8], yet the utility of a drug like amitriptyline can vary significantly depending on the specific condition being treated [9, 10]. This variability creates a clinical imperative to rigorously evaluate established therapies within distinct patient populations, such as those with leprosy-related neuropathic pain, to refine treatment guidelines and avoid therapeutic futility [2].
