Arterial Netrin-4 Maintains Adult Sympathetic Neurons in Mice

Maintaining neuronal integrity throughout adulthood is essential for physiological homeostasis, yet the mechanisms preventing age-related neuronal loss remain incompletely understood. While clinical attention often focuses on central neurodegeneration in conditions like Parkinson's disease, where a meta-analysis of 11 trials involving 210 patients suggests cell therapy may improve motor function in the "off" state [1, 2], the peripheral autonomic nervous system also undergoes significant attrition that impairs organ function [3, 4]. Immunosenescence (the age-related dysfunction of the immune system) and chronic inflammatory states further complicate this landscape, often exacerbating the decline of cellular survival signals through pathways such as the overactivation of nuclear factor kappa-light-chain-enhancer of activated B cells (NF-κB) [5]. Current interventions, including peripheral nerve stimulation or stellate ganglion blocks which can reduce pain scores by a weighted mean difference of 6.24 mm (p = 0.019), primarily target symptom management rather than the underlying survival pathways of these fibers [6, 7]. Shifting focus to these foundational survival mechanisms, a recent murine study identifies a neurovascular survival axis where arterial smooth muscle cells (aSMCs) secrete netrin-4 (Ntn4) to maintain superior cervical ganglion (SCG) sympathetic terminals, preventing a caspase-independent death program linked to receptor-interacting serine/threonine kinase 1 (RIPK1) [8].
